Equipment Service Log Template
Track repairs, maintenance, and downtime for every piece of equipment in your organisation.
Purpose
This equipment service log helps maintenance teams track every repair, preventive maintenance activity, and downtime event for critical equipment. Each entry links to the asset record so you can identify recurring failures and make data-driven replacement decisions.
Log Entry Fields
- Equipment ID / Asset Tag: Unique identifier from your asset registry.
- Equipment Name & Model: Descriptive name and manufacturer model number.
- Date of Service: Date the maintenance or repair was performed.
- Service Type: Preventive maintenance, corrective repair, emergency repair, or AMC visit.
- Issue Description: Problem reported by the user or identified during inspection.
- Root Cause: What caused the issue — wear and tear, user error, environmental factor, or defect.
- Action Taken: Steps performed to resolve the issue. Include replaced parts and adjustments made.
- Technician: Name of the internal technician or external service provider.
- Spare Parts Used: List of replaced components with part numbers and costs.
- Total Cost (INR): Labour + spare parts + any third-party charges.
- Downtime Duration: Hours the equipment was non-operational.
- Resolved: Yes / No. If no, link to open ticket or follow-up schedule.
Dashboard Metrics to Track
- MTBF (Mean Time Between Failures): Average operating time between failures. Low MTBF indicates poor reliability or aging equipment.
- MTTR (Mean Time To Repair): Average time to restore service after failure. High MTTR suggests skill gaps or spare parts issues.
- Cost Per Equipment: Total maintenance cost divided by equipment count. Identifies assets that cost more to maintain than replace.
- Recurring Issue Count: Number of times the same problem has been logged. More than 3 in 6 months = root cause not addressed.
Review Cadence
Review this log monthly during the operations standup. Flag equipment with declining MTBF or rising costs for replacement planning before they cause critical downtime.
