Asset Transfer Form
Standardised form for transferring IT assets between employees or departments.
Purpose
This form documents every asset transfer between employees, departments, or locations. It creates an audit trail and ensures the receiving party acknowledges the transfer.
Section 1: Asset Details
- Asset Tag / ID: Unique identifier from the asset registry
- Asset Type: Laptop, desktop, monitor, phone, peripherals
- Make & Model: Manufacturer and model number
- Serial Number: From the physical device
- Current Condition: Working, damaged (describe), under repair
Section 2: Transfer Details
- From (Current User): Name, department, employee ID
- To (New User): Name, department, employee ID
- Reason for Transfer: Role change, department move, replacement, other
- Transfer Date: When the asset is physically moved
- Location Change: From location/building to new location/building
Section 3: Accessories
List all accessories transferred with the asset: charger, docking station, peripherals, carrying case, etc.
Section 4: Acknowledgment
Both parties sign acknowledging: asset condition at transfer, accessories received, and responsibility transfer date.
