Server Maintenance Log Template

A detailed log for tracking server maintenance activities, patch deployments, hardware changes, and performance trends.

Purpose

A server maintenance log provides a complete historical record of every action taken on your servers. This is essential for: troubleshooting recurring issues, proving compliance during audits, planning hardware refresh cycles, and onboarding new IT team members who need to understand server history.

Server Information

  • Server Name / Hostname: Unique identifier within the network.
  • IP Address: Management IP and any additional IPs.
  • Physical / Virtual: Physical server or virtual machine. If VM, note the hypervisor host.
  • Role / Purpose: Domain controller, file server, application server, database server, backup server, monitoring server.
  • Operating System: OS name, version, edition, build number.
  • Hardware Specs: CPU model and cores, RAM capacity, storage configuration (RAID level, disk count, capacity), NIC speed and count.
  • Location: Rack number, RU position, data centre / server room name.

Maintenance Log Entry Template

Review Cadence

Review the server maintenance log monthly: identify any server requiring frequent interventions (>1 maintenance event per month) for root cause analysis and potential replacement planning. Archive logs quarterly for compliance retention.
